Prior to your appointment, please contact your insurance carrier and make sure that a Finger Lakes Women's Health doctor is listed as your OB/GYN or alternate primary care provider. Please arrive 10 minutes prior to your appointment and bring your insurance card with you to your first visit. If you can not attend your scheduled appointment with us, please allow us 24 hours notice when canceling. It is also our office policy to reschedule a patient if they are more than 15 minutes late for their appointment.

Although we work very hard at running on time, an OB/GYN practice is full of urgent and emergency problems, and even though we leave time in our schedules to accommodate such problems, we will sometimes be unavoidably late. We try our best to minimize lateness.

Our office will handle all calls during regular business hours. Calls involving more than minor problems will be handled by the Physician Assistant, CNM, Nurse or other staff members who are trained to deal with these issues. Please leave phone numbers where you can be reached that day. Your phone call is important to us.
For truly urgent problems, the doctor will be available at any time. After office hours contact one of the offices and the answering service will contact the doctor on call. If for some reason you are unable to contact our office, our after hours answering service may be reached directly at 585-327-2180.

We do not release information about our patients under any circumstances unless we have an authorization from the patient to disclose information. This also applies to phone calls from worried husbands and mothers. Unless we have permission of the patient, we are not authorized to speak to anyone. Please understand that this policy complies with both the requirements of the Privacy legislation as well as the ethics of good medical care.

Please request that your pharmacy fax an authorization for prescription refill to our office. We will confirm the prescription is correct and authorize the appropriate number of refills. This is to ensure that errors do not occur when managing the refill request. Please allow 24-48 hours for refills to be completed. We require all patients to be seen at least on a yearly basis for most prescriptions.

Payments and co-payments must be rendered at the time of the visit. We accept VISA, MASTERCARD and American Express.

If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. This courtesy, on your part, will make it possible to give your appointment to another patient. Please note that any patient not showing for their appointment more than once will have a fee of $20 and be discharged after the third occurrence.

Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.
